Admissions Process

Making the decision to admit to a rehabilitation or nursing facility can be a stressful and time-consuming process. It often comes at a time when families are taking part in a major change that has occurred in the life of their loved ones. At Cedarview, a dedicated admissions team works closely with patients and families, to process all aspects of admission including helping secure whatever benefits a resident may be eligible to receive from Medicare, Medicaid or other insurances. The Admissions team will assure that the entire process is a pleasant experience, thoroughly explain and alleviate the stress often incurred when admitting a loved one to a healthcare facility.

A great deal of background work and preparation is done by our professionals prior to and upon admission to assure that a resident is given the best care from the very first moment of arrival. This includes a complete evaluation of the resident’s medical history, an assessment of therapeutic needs, as well as an analysis of lifestyle such as nutrition, exercise, and social needs. Once admitted to Cedarview, the entire team of clinicians will assess, evaluate and collaborate, to design a carefully crafted individualized plan of care with specific objectives and goals outlined, to ensure that every aspect of a resident’s needs are addressed and met. Throughout, Social Service will maintain close contact with the resident and family to continuously work toward Cedarview’s goal of providing excellent care and patient satisfaction, throughout your journey along the road to recovery.
